It seems there are a number of companies developing applications that can be used in the field to collect data, particularly in developing countries using USAID funds. A couple of graduate students at UT Austin used this application (re programmed) for a survey of customers who purchase food at those corner markets you have here in America. Maybe this type of application can be adapted for data collection work done in communities with public health problems.
