INFOGRAPHIC: 2012 Population Health Management Survey Findings-Health Intelligence Network

The term population health once referred almost exclusively to public health.  With health reform's emphasis on health care system transformation facilitated by health information technology (HIT),  use of the term migrated to health care services delivery, employer health benefits, and health plans.  The infographic below summarizes findings from  2012 Healthcare Benchmarks: Population Health Management.   

The Health Intelligence Network promotes the study as follows:  "An in-depth analysis of population health management (PHM) trends, including prevalence of PHM initiatives, program components, professionals on the PHM team, incentives, challenges and ROI. This resource analyzes the responses of 102 healthcare organizations to HIN's industry survey on PHM trends, administered in September 2012. It delivers the latest metrics and measures on current and planned PHM initiatives, providing actionable data on the most effective PHM tools and workflows, risk identification strategies, tools to boost health plan member and consumer engagement, modalities for program delivery, and much, much more."
